Health care transition is the process of changing from a pediatric to adult model of health care. At the Brigham and Women’s Hospital Endocrine Transition Care Clinic, we hope to make your move from pediatric to adult care as seamless as possible to keep you on track for long-lasting wellness. We will arrange careful care coordination between yourself, your pediatric endocrinologist, and an adult endocrinologist here at Brigham and Women’s Hospital.
