Register at Brigham and Women’s Hospital

Before scheduling your appointment, you will need to register with Brigham and Women’s Hospital if you have not done so already.

To register, please call the BWH Registration Line: (866) 489-4056.

You should have the following information available when you call:

  • your name
  • address
  • date of birth
  • social security number
  • insurance information
  • emergency contact information
  • phone number(s)
  • email address
Preparing for your first appointment

Our coordinator will reach out to you to help you get set up with your first appointment via phone and/or email. Your pediatric doctor may have provided your address, telephone number and an email by which we will try to contact you.

If you get a message from the email address endotransition@bwh.harvard.ed or with the subject line “Brigham and Women’s Transition Care Clinic”, that’s us!

Once your appointment is scheduled you will receive a welcome packet via email or regular mail. Your welcome packet will include:

  • information about our program
  • directions to clinic
  • instructions for how to sign up for our online patient portal “Partners Patient Gateway”
  • a questionnaire to help us get to know you better: this may be sent to you to fill-out online via separate secure email, or if you do not have access to a computer please fill out a copy that will be mailed to you and bring it to our first appointment

Your pediatric endocrinology team will have sent information to your new doctor, but if there is any additional information you would like to ensure we have, please bring it to your appointment. Make sure you have an updated list of your medications, vitamins, and supplements. If you have diabetes, bring your meter, CGM reader, and/or logbook.

Please feel free to come to your appointments with a family member or anyone in your life who provides support. We will respect your privacy and love when our patients are supported in their health care.

Our coordinator will contact you to remind you of your upcoming appointment and answer any lingering questions.

After your first appointment

We will send out a brief survey for you to let us know how your visit went. This is completely anonymous and helps us to improve our care.
